Google Ads for Chiropractors: Settings That Waste Budget

Google Ads for chiropractors earn their keep on local searches and counted first visits. Six settings that waste a small clinic's money, and the fix.

In Google Ads for chiropractors, look for waste first in settings nobody at the clinic chose: a location option, two network boxes, recommendations someone accepted. The ads earn their keep when they buy local searches from people ready to book and you count the first visits they bring.

Quick answer

  • Set location options to "Presence" so ads show to people in or regularly in your area.
  • Sort the search terms report by cost, and block searches you would not book as negative keywords.
  • Turn off auto-apply recommendations and any AI Max setting you did not choose.
  • Count calls past your own minimum length, online bookings and first visits from your records.
  • Keep patient lists, remarketing lists and lookalike segments out of health campaigns.

Six places a new campaign leaks money

SettingWhat Google doesWhy it costs a small clinicWhat to do
Location optionsThe default reaches people in your area and people interested in itSomeone researching your city from elsewhere can click"Presence"
Search partners and DisplayNew campaigns are set to show across Google's whole networkClicks come from other sites, not Google SearchUntick both, or compare by network first
Phrase matchWider since 2021 than its old definitionSearches that share your keyword's meaning trigger the adCheck the search terms report weekly and add negatives
AI MaxSwitches on text customization and final URL expansionGoogle writes ad text and picks landing pagesOff unless you chose it
Auto-applyApplies recommendations someone opted intoChanges land without you reading themUncheck all
Call lengthCounts calls past a minimumA minimum set too low counts wrong numbersUse the length of a real booking call

Google's location help calls "Presence or interest" the default and recommended option. The same page says to consider Presence when a business "falls within a sensitive vertical with strict targeting limitations."

Health is on Google's list of sensitive interest categories with targeting restrictions, so start with Presence. Google's page on out-of-area clicks shows where to switch.

Google's definition of the Google Network says new campaigns are set up by default to show ads across the entire network.

In a Search campaign, the Google Ads Editor help names the two boxes that control this, "Include search partners" and "Display Network". Search partners are hundreds of non-Google sites plus YouTube. When I was buying Google Ads for other businesses, these two boxes were the first thing I read.

Since 2021, phrase match also catches searches Google judges to share your keyword's meaning.

On AI Max, Google says turning it on turns on search term matching, text customization and final URL expansion. You can switch the last two off one by one. With final URL expansion on, pinned headlines are not used when Google picks another page.

Auto-apply is opt-in, so anything ticked was ticked by someone. Go to Recommendations, then Auto-apply settings, uncheck each item, then select Save. The History tab there shows when each one was turned on, and Change history shows which user opted in.

What Google's health rules mean for a chiropractic clinic

Chiropractic is not on the restricted list in Google's healthcare and medicines policy, which names topics like prescription drug services, addiction services and health insurance. Speculative or experimental treatments are not allowed at all. Its other limits are in Google's health ad rules for practices.

Google's misrepresentation policy also bans ad text that promises an improbable result as the likely outcome, or uses accidents or illness to pressure people into acting now.

An ad about back pain or sciatica names a physical health condition, so plan as if this limit applies to your campaigns. That rules out uploading past patients to find similar people. It also rules out remarketing to your site's visitors, which Google calls "your data segments".

Medicare limits coverage to manual manipulation of the spine to correct a subluxation, and treats continuing care after a condition stabilizes as maintenance therapy it does not cover. Ad copy that says "Medicare covers chiropractic care" goes further than that page does. Free or discounted first visits fall under a federal rule of their own.

In Ontario, the College of Chiropractors' Standard of Practice S-016, Communication to the Public covers any public message promoting chiropractic services, websites and social media included. It bans any express or implied guarantee of success, and makes the chiropractor responsible for messages an employee or third party produces for them. Other provinces have their own college rules.

Which searches to buy

Start with "chiropractor near me" and city searches. Google's Keyword Planner puts a top-of-page click on "chiropractor near me" at $2.78 to $15.97 (October 2026). Divide your monthly budget by both ends of that range to see how many clicks it buys before any other search gets a dollar.

Symptom searches such as "lower back pain" also catch people who want exercises or an MRI, not a chiropractor. Run them as a separate campaign so their cost per first visit shows on its own, and check their click price for your city instead of assuming it is lower.

Auto-accident searches, if you take those cases, belong in a third campaign. Those visits are paid differently, so count their first visits on their own.

Count first visits from your records

A call from an ad can count as a conversion when it lasts longer than a minimum you set. Google's developer documentation gives 60 seconds as the default for "Calls from ads". Google's help page for call conversions in the Ads screens names no default, so open your call conversion action and read the number.

That help page adds that with call recording on, Google's AI rates each call from the recording instead. When recording is off or not possible, it falls back to your set length.

For online bookings, booking conversion tracking shows how to count the confirmed booking page, not the click on "Book". For first visits, the practice record is the source, so your chiropractic software needs to log the ad source on each new patient's file. First visits are where the ad's job ends; whether patients finish a care plan is a later count in the same records.

Check your own campaign in six steps

  1. In Campaign Settings, open Locations, then Location options, and pick Presence.
  2. On the same page, under Networks, note whether the search partner and Display boxes are ticked. Untick both for a small budget.
  3. Under Recommendations, open Auto-apply settings. Uncheck everything, then save.
  4. In the Asset optimization panel, if text customization or final URL expansion is on and you did not choose it, switch it off.
  5. In the Search terms report, sort by cost and note each search you would not book. Then add each one as a negative keyword.
  6. Under Goals, open the "Calls from ads" conversion action and check its call length. Make sure calls are not the only action counted.

Questions owners ask

Frequently asked questions

Can a chiropractor run Google Ads without a restricted-healthcare certification?

For ordinary chiropractic care, yes. Google's healthcare and medicines policy lists restricted topics such as prescription drug services, addiction services and health insurance, and chiropractic is not among them. Speculative or experimental treatments are not allowed at all. Your ad claims and audience settings still follow Google's other health rules.

Should I switch on AI Max for my chiropractic campaign?

Only on purpose. Turning on AI Max also turns on text customization and final URL expansion, which let Google write ad text and send clicks to other pages of your site. Each can be switched off in campaign settings, and a clinic with a short list of pages is usually better off sending clicks to the page it chose.

How long should a call last to count as a patient call?

Count calls that last long enough to be a booking conversation. Google's developer documentation gives 60 seconds as the default minimum for calls from ads; the help pages for the Ads screens do not state one. Check the number in your own conversion action, then listen to a sample of calls at your desk to see how long a booking takes.

Can I upload my patient list to find similar people?

Not for ads about back pain or other health conditions. Google's personalized advertising policy puts physical health conditions and their treatments in its Health category, and advertisers promoting in it can't use Customer Match or lookalike segments. Ask your agency to confirm every audience in the account is a predefined Google audience, such as in-market segments or location targeting.

Do ads for auto-accident patients belong in the same campaign?

Give them their own campaign. Accident cases are paid differently from a routine back-pain visit, so a first visit is worth a different amount. A separate campaign lets you read each one's cost per first visit on its own, and Google's misrepresentation policy bars ad text that uses accidents to pressure people into acting now.
