CallRail is call tracking software: it gives your website, ads and listings their own phone numbers that forward to your real line, logs every call to those numbers with the marketing source behind it, and can record and transcribe the conversation (CallRail). For a Google Ads advertiser it does one more job: when someone who clicked your ad calls a number CallRail swapped onto your website, it sends that click's id to Google Ads as a conversion. How does CallRail work?
A source tracking number is one fixed number per campaign, such as a mailer or a directory listing; a website pool is a set of numbers handed out to website visitors (types). CallRail's JavaScript snippet swaps a pool number onto your page and keeps the visitor's source in a cookie (DNI). Each pool number belongs to one visitor at a time, so a call to it carries that visit's source, paid keyword, landing page and pages viewed (visitors).
What does CallRail send to Google Ads?
The click id of the ad visit: a gclid, or a wbraid or gbraid on some iPhone traffic. CallRail posts it to the action its integration created, "Phone Call", or "First Time Phone Call" and "Repeat Phone Call" if you split them, with the source "Import from clicks" (integration). Two extras are opt-in. With enhanced conversions for leads on, it adds hashed caller details (enhanced conversions). With "Send a call's value to Google Ads" on, it waits a delay you set, from 4 hours to 30 days, and sends the value once, so the delay has to cover the time your front desk takes to qualify calls (values).
What stays in CallRail?
The keyword, the landing page, the pages viewed and the front desk's lead status. CallRail's own healthcare page warns that a call to a tracking number tied to a specific campaign, or to a visitor and their search keywords, starts to reveal the caller's medical needs (Healthcare accounts). Keeping those details out of Google is the boundary a HIPAA-conscious setup draws; for a medical practice I would also leave enhanced conversions for leads off, so the caller's phone never leaves the practice.
Which calls does CallRail never send to Google Ads?
Only Google Ads calls to the website pool post (integration). Calls to a source tracking number, such as one on your Business Profile, are logged and labeled but never posted (source tracking, Business Profile). Call-asset calls that ring a CallRail number through Call Details Forwarding are logged too, while Google counts them under its own call conversion settings (CDF). Does CallRail's "source" name the ad behind this call?
Not always for a repeat caller. The call log and reports typically show the contact's First Touch milestone for source, campaign, medium and landing page (Call Log). In CallRail's own example, a lead who first called after a Facebook ad calls again 18 months later after clicking a Google ad, and under First Touch the Google ad gets no credit. The First Touch and Last Touch columns show both, but neither is shown by default (attribution modeling).
Is a "first-time caller" a new patient?
Not necessarily. CallRail tells callers apart by caller ID, so one person calling from two phones counts as two callers (Unique Callers), and it only sees calls to its own numbers. A long-standing patient who always dialed an untracked main line is a first-time caller the day they call a pool number. Read "First Time Phone Call" as new to CallRail, not new to the practice.
Does CallRail know whether the caller became a patient?
Only up to the booking. Conversion Signals, part of Premium Conversation Intelligence, can tag a call booked or not booked from its transcript (Conversion Signals). On a Healthcare plan with the NexHealth add-on and a signed BAA, the EHR integration tags a call when a new appointment for the caller's phone number is booked during or shortly after it (EHR integration). Neither knows whether the patient came in; that comes from joining each call to the visit that followed. In the patient tracking I built, a phone-booked appointment can only be matched to a call if the front desk records the caller's phone.
What does CallRail leave for you to decide?
Which calls count: every Google Ads call to the pool posts, new patient or wrong number, unless you create an integration filter, for example by number or tag (integration filters). And which action Google bids on. CallRail's "Phone Call" counts website-pool calls, and Google's "Calls from ads", in the Conversions column by default, counts calls through Google forwarding numbers, so the two never share a call and both can stay Primary (integration, measure calls from ads).

The first row decides the rest: each tool counts only the numbers it controls. My rule of thumb: if most calls your ads produce come through call assets and call-only ads, and nothing else you spend on rings the same line, skip CallRail, because "Calls from ads" can already count those calls and CallRail would post none of them. Buy it once a directory, a mailer, paid social or a Business Profile you work on shares the phone, since those calls land in one log only if CallRail owns their numbers, or when every website visitor, not only ad clickers, should see a tracked number (website pool, call conversion types). Then connect CallRail to Google Ads and set which call actions are Primary before its first call posts.

What is CallRail used for?

Does CallRail track calls from Google Business Profile?
It logs them if the profile shows a CallRail tracking number, and its Business Profile integration labels them as such. It does not send them to Google Ads as conversions. Calls to a main number on the profile that CallRail does not own never reach CallRail at all.
Does CallRail send calls from organic visitors to Google Ads?
No. Only calls to the website pool from visitors whose source is Google Ads report as conversions. If your pool also tracks organic visitors, their calls are logged in CallRail without a keyword, because none is passed for organic traffic.
Can I change a call's value after CallRail sends it to Google Ads?
Not in Google Ads. CallRail sends a call's value only once, after the delay you set, so a value corrected later stays in CallRail. If your values tend to change, CallRail recommends a longer delay.
